
The Story
The Galaxy Watch Ultra 2 and Watch 9 show you a wall of health numbers. Heart rate, an ECG, body composition, blood oxygen, sleep stages, a stress score, a five-second “antioxidant” reading, a blood pressure figure, a sleep apnea check. It looks like a clinic on your wrist. Here’s the thing though: those readings don’t all mean the same thing, and they don’t all carry the same weight. Some are cleared by a regulator to flag a real medical condition. Some are wellness estimates that a doctor would never act on. And a couple are genuinely new this generation, while others have quietly been on Galaxy Watches for years.
So let’s actually walk through it — sensor by sensor, feature by feature — and be honest about where each one sits. I wrote a general primer on how smartwatch health sensors work, and this is the Samsung-specific companion to it: what the Ultra 2 and Watch 9 do with the hardware Samsung packs in.
Everything on these watches routes through one piece of hardware Samsung calls the BioActive sensor. It’s not a single sensor — it’s a stack that does three physically different jobs. There’s the optical bit (green, red, and infrared LEDs that read your pulse and blood oxygen by shining light into your skin), the electrical bit (metal contacts that read your heart’s actual electrical signal for ECG, and pass a tiny current through your body for body composition), and — a capability that started shipping on the Watch 8 and carries forward here — a multi-wavelength spectroscopy bit that reads pigment in your skin. Same slab of glass and contacts on the back of the watch, three different measurement principles. Keep that split in mind, because it’s the difference between a real electrical reading and an optical estimate.
### The heart stuff: PPG, ECG, and where the regulator draws the line
Your continuous heart rate comes from PPG — photoplethysmography, the green light on the underside. It’s genuinely reliable at rest and decent for steady cardio, but it’s an optical guess that motion throws off. That’s a lifestyle sensor. It’s not what’s cleared.
The ECG is. When you touch the frame with a finger you complete a circuit and the watch records your heart’s actual electrical rhythm — a single-lead ECG, a simplified cousin of the twelve-lead machine in a hospital. Samsung’s ECG and its companion Irregular Heart Rhythm Notification are FDA-cleared, and the thing they’re cleared to do is narrow: they’re authorized to flag signs of atrial fibrillation, the most common serious arrhythmia. Both are for users 22 and older. Neither detects a heart attack. The IHRN feature quietly watches your background pulse and, if it sees a run of irregular readings, nudges you to sit down and take a proper ECG. That’s the model to understand: the passive optical sensor raises a flag, and the electrical sensor does the real check.
What’s genuinely new on this generation is a cluster of scoring features Samsung frames as its own: a Heart Health Score, a Fitness Index, and Daily Cardio Load. None of them is a new sensor — they’re the watch synthesizing data it already collects into a single number. Heart Health Score rolls activity, sleep, body composition, and stress trends into one cardiovascular wellness read with lifestyle tips. Fitness Index uses your body composition and workout history to gauge your overall fitness level and set personalized goals. Daily Cardio Load looks at how hard you trained and estimates how much recovery you need before the next session. All three are wellness summaries, not cleared diagnostics — useful as personal trend lines, not numbers to bring to a cardiologist.
Worth flagging the one that carried over and is now moving in reverse: Vascular Load, introduced back on the Watch 8, gives an overnight read on the strain your cardiovascular system is under. It’s a wellness metric, never a cleared one. And in the US it’s actually being pulled — Samsung is removing Vascular Load from US Galaxy Watches with the One UI 9 Watch update, apparently over FDA compliance, and replacing it with a cuff-calibrated “Blood Pressure Trends” feature instead. International models keep Vascular Load; US models don’t. A good reminder that “new health feature” and “available to you” aren’t the same thing.
### Blood pressure: the one to be most careful with
Samsung will show you a systolic and diastolic number, and it’s the reading I’d trust least out of the box. A real blood pressure monitor squeezes your arm with a cuff. A watch can’t do that, so it estimates pressure from the shape of your pulse wave through the optical sensor — and that only works if you first calibrate it against a real arm cuff, then re-calibrate roughly every 28 days. In the US, Samsung rolled the feature out under the FDA’s general-wellness policy — meaning it’s explicitly not cleared as a diagnostic medical device, unlike the ECG. Independent research has repeatedly found cuffless wrist blood pressure falls short of clinical accuracy. Read it as a rough trend, never as a reason to change medication.
### Body composition, blood oxygen, temperature, stress
Body composition (BIA) uses that tiny electrical current to estimate body fat, muscle mass, and body water in a few seconds. It’s a reasonable ballpark that trends well over time, but it drifts with hydration and time of day — weigh it as a direction, not a verdict.
Blood oxygen (SpO2) uses red and infrared light to estimate how saturated your blood is. Useful context for altitude or sleep, but it’s a wrist estimate, not the fingertip clip a nurse uses, and it reads less reliably on colder skin, poor fit, or darker skin tones where the optical signal is weaker.
Skin temperature comes from an infrared sensor, and here’s the part people misread: it is not a fever thermometer. It’s tuned to catch the small overnight shift in wrist temperature that tracks a menstrual cycle, which feeds cycle and ovulation estimates. Relative change over nights — not an absolute “you have a fever” number.
Stress isn’t a dedicated sensor at all. It’s an algorithm reading your heart-rate variability — the tiny beat-to-beat timing changes — and mapping that to a stress score. Directionally interesting, easily thrown off by caffeine, movement, or a bad night’s sleep.
### Sleep, and the feature that actually clears a bar: apnea detection
Sleep tracking gives you stages, a sleep score, snore detection, and a Bedtime Guidance coach — introduced on the Watch 8 and carried forward here — that suggests a consistent sleep window. The stages and score are the usual wearable estimate: helpful for spotting patterns, not a lab polysomnogram. (I went deep on reading smartwatch sleep numbers separately.)
The one sleep feature that clears a real regulatory bar is Sleep Apnea detection. It has FDA De Novo authorization (DEN230041) — the pathway for a genuinely new low-risk device class — and it’s designed to flag signs of moderate-to-severe obstructive sleep apnea in adults 22 and older. To use it you track sleep on two nights, each over four hours, within a ten-day window. Two caveats Samsung states plainly: it’s not a diagnosis and doesn’t replace a clinician, and it’s not for people already diagnosed with OSA. It’s a screening nudge to go get checked — which, for a condition that goes undiagnosed for years, is genuinely useful. And it isn’t on everywhere: the US clearance is the FDA De Novo, and a handful of other markets (South Korea and Japan among them) offer it under their own local regulatory approvals, because each region’s regulator has to sign off separately.
### The five-second party trick: Antioxidant Index
Carried over from the Watch 8, the Antioxidant Index is the one that looks like magic. Take the watch off, press your thumb to the back sensor for five seconds, and it returns a 0–100 score. What it’s actually doing is multi-wavelength spectroscopy — reading how your skin absorbs different colors of light to estimate carotenoid levels, the pigments from colorful fruits and vegetables that build up in skin. It’s a rough proxy for whether you’re eating your vegetables, and Samsung is clear it’s a wellness feature, not a diagnostic one. Fun, motivating, and firmly in the “don’t overthink it” bucket.
### Running Coach
Also carried forward and worth a mention because it’s the fitness feature that punches above its weight: Running Coach. Run outdoors for twelve minutes and it rates your running level on a 1–10 scale, then builds a training program around it. It’s not a sensor claim — it’s the watch turning your heart-rate and pace data into structured coaching, and it’s one of the more genuinely useful additions for anyone actually training.
The Takeaway
Here’s the mental model that makes this whole list make sense. Sort every feature into three buckets. Cleared — ECG, irregular rhythm notification, and sleep apnea detection. These went through the FDA, they’re age-gated at 22+, and they’re built to flag one specific thing each (AFib, AFib, and moderate-to-severe OSA). Trust these to raise a flag, then see a doctor. Wellness estimates — blood pressure, body composition, SpO2, stress, Vascular Load, Antioxidant Index. Useful as personal trend lines, useless as diagnoses, and in blood pressure’s case explicitly shipped under a wellness policy rather than cleared. Lifestyle context — continuous heart rate, sleep stages, skin temperature. Good for patterns, not precision.
Notice what’s actually new this generation versus what’s just… there. The genuinely fresh additions on the Ultra 2 and Watch 9 are the scoring layer — Heart Health Score, Fitness Index, and Daily Cardio Load — which synthesize existing sensor data into single numbers. Vascular Load, Bedtime Guidance, the Antioxidant Index, and Running Coach all arrived on the Watch 8 the year before and simply carried over (and in the US, Vascular Load is actually being removed). And the headline clinical features — ECG, AFib alerts, blood pressure, sleep apnea — mostly landed on older Galaxy Watches by software update and got inherited here. Which lines up with something I keep coming back to: on this generation, the real change was the chip swap, not the health list. If a specific health feature is your reason to buy, check whether your current Galaxy Watch already got it in an update first.
And the honest framing on all of it: none of these features diagnose or treat anything. The cleared ones are screening tools that tell you when to see a professional; the rest are wellness numbers that are more interesting as trends than as absolutes. A watch that nudges you toward a real diagnosis is doing something valuable. Just don’t confuse the nudge with the diagnosis.
